Certificate of Origin / GSP Form A
Certificate of Origin is an independent document evidencing the origin of the goods or manufacturer. It contains the description of goods, quantity or weight, value of goods shipped (only for application, it will not be indicated on the original official certificate ) and the producing area. Two kinds of Certificate of Origin are generally adopted in our country: Certificate of Origin which is issued by CIQ (China Exit and Entry Inspection and Quarantine Bureau) or CCPIT (the China Council for the Promotion of International Trade) and Generalized System of Preference Certificate of Origin Form A (GSP Form A) which is issued by CIQ. GSP Form A can be used to get preferential import duties in many countries, such as New Zealand, Canada, Japan, EU members, etc.

The Main Contents and Notes of Certificate of Origin 产地证的主要内容及注释：
1.Certificate No. 证号
The certificate is invalid if the column is blank. 本栏若为空，产地证无效。
2.Exporter 出口方
It is the benefitiary. The exporter’s address is asked to be made out in details including street, doorplate number; city and country. 出口方是受益人，其地址要详细填制，包括街道、门牌号、城市和国家。
3.Consignee 收货人
It is usually the buyer or the notify party of the B/L. If it is required to be in blank, “To whom It May Concern” or “To order” is marked here. 通常是买方或提单通知方。如果要求空白，这儿标为“相关人”、“按指示”。
4. Means of transport and route 运输方式和路线
The port of destination and shipment, terms of shipment are to be made out. If transshipment is allowed, the place of transhipment is marked. The following is an example.
FROM QINGDAO TO ROTTERDAM VIA HONG KONG BY VESSEL. 列明装运港、目的港、装运条件，如果允许转船，标明转船地点，以下是例子：
船运从青岛经香港转船至鹿特丹
5. Country / region of destination 目的国/区
It refers to the country or region at which the goods arrive, the middle businessman is not mentioned. 指货物到达的国家和地区，中间商不提及。
6.For certifying authority use only 签证机关专用栏
It is usually kept in blank. The content is made out by the issuing organization according to the concrete condition. 通常空白不填。由出证机构根据具体情况填入相关内容。
7.Marks and numbers 标记及号码
To be made out according to the invoice, the phrases “as per invoice No.xxx” or “As per B/L No.xxx” are not allowed. “N/M” is filled in in while there is no mark. 根据发票填制，短语“根据发票号：xxx”或“根据提单号：xxx” 是不允许的。没有标记的时候，填入“N/M”。
8. Number and kind of packages; description of goods 包装件数及包装种类；货物描述，如：

说明：在货物描述结束时应该有终止符“********”
9.H.S. Code H.S.编码
The part should be kept in conformity with customs declaration. 本部分应与报关一致。
说明：按照商品在《商品名称和编码协调制度》(Harmonized Commodity Description & Coding System)中的编码填写。
10.Quantity 数量
The quantity and measurement are required to be printed, for example: 要求打印上数量及尺码。

N.W.1,000MT or 1,000MT (N.W.) 如：100套；净重1,000MT。
11.Number and date of invoice 发票号和发票日期
Make out according to the invoice. 按发票填制。
12.Declaration by the exporter 出口方申明
The issuing place and date are printed here with the exporter’s stamp and signature. The date is not later than the date of B/L and not earlier than the date of invoice. 出证地点和日期打印于此，出口商签字盖章。日期不得迟于提单日期，不得早于发票日期。
13.Certification 证明
The issuing place and date are to be made out. The issuer should sign manually with seal. 该出证地点和日期填制，出证人应手签字并盖章。
14.For official use 供官方使用
若货已出口，加盖“ISSUED RETROSPECTIVELY”(后发证书)红色印章。若证书丢失或被盗，“THIS CERTIFICATE IS IN REPLACEMENT OF CERTIFICATE OF ORIGIN NO.XXX DATED XXX WHICH IS CANCELLED”(本证书替代已取消的XXX号XXX日出的产地证)用红色标记。
15.Item number 项号
如果同一批货物有不同的货物品种，应根据品种不同标注“1”、“2”、“3”……，如果只有一批，必须标注“1”。
